may not provide coverage
while out of the country for
medical costs incurred in mission travel.

Accidental Death and Dismemberment (Limit $1,000,000 per
team)
To Insure your Team
Send HOPE office required form, along with one check
( payable to HOPE) to cover the cost of
insurance
$1.50 per person per day and registration ($15 per
person) for the entire team.
The period of coverage includes date of departure and date of
return.
Please include a list of everyone's full name and email
address.
Please
indicate who the team leader is along with the team leader's street
address, email address and telephone number.
THESE FORMS
MUST BE RECEIVED BY THE HOPE OFFICE TWO WEEKS PRIOR TO THE TEAMS
DEPARTURE!
Once received by the HOPE office and processed, the CMA
Agency will send the team leader certificate
of insurance with terms of coverage.
HOPE will remit your team’s payment to CMA
for total premiums due.

TEAM LEADERS PLEASE NOTE:
Insurance coveres: medical cost incurred after
accidents, but does not include medical costs due to illness.
However, a rider is included that provides for medical evacuation in
the event of any serious medical emergency or
repatriation in the event of a fatality. This service is
provided by Assist America, Which will handle all arrangements
and pay all costs after proper
consultation.
Please take along several copies of
the “Accident or Sickness Claim For” in case there are
accidents. The back of this form must be filled out by the
attending medical personnel where the persons are treated, and
you will pay the cost of treatment and be reimbursed by CMA.
(This form is sent to you upon
registration.)
If team members wish to have
additional insurance for medical costs due to illness, please
contact HOPE directly for complete information. In this case,
premiums will be paid directly to CMA Agency, Inc. and will
not go through the HOPE office. Your current Insurance Company may or may not
provide coverage while out of the country for medical costs
